At Accerta, we believe everyone deserves a fair opportunity to access the care and supports they need to live healthier lives. That belief is at the heart of the Accerta Health Access Foundation, which supports organizations breaking down barriers and making a difference in communities. We’re proud to introduce the five organizations receiving a share of $150,000 through the Foundation’s 2025–26 Health Access Grant, helping bring more care, support and opportunity to the people who need it most.
A foundation built on giving back
The Accerta Health Access Foundation is a registered charitable foundation created by Accerta, a proudly Canadian-owned and operated B Corp-certified social enterprise, that partners with the public sector to deliver health and social programs. Through the foundation, Accerta reinvests in the communities where we live and work, supporting projects that improve access to care, remove barriers and promote the health and well-being of communities.
Each year, the foundation provides $150,000 in grants to registered charities working to address gaps in access to care and strengthen the health and well-being of the communities they serve. Since its launch, the foundation has awarded $450,000 in grants, and with this year’s recipients, that investment will reach $600,000.
Meet the 2025-26 grant recipients
This cohort reflects what health access truly means. From gender-affirming care in Northern Ontario to overnight dementia respite, counselling for people with access barriers, accessible medical transportation, and round-the-clock crisis care, these five organizations are meeting people where they are:

| CHATS – Community & Home Assistance to Seniors | CIBPA Place An overnight respite home that provides short-term residential stays for individuals living with dementia. It offers a safe environment with 24/7 staffing, personalized care, nutritious meals, and structured social and wellness activities, giving caregivers peace of mind knowing their loved one is supported, cared for and receiving the medical care required. |
| Hope House Guelph | Low-Barrier Mental Health Counselling Free, low-barrier mental health counselling for individuals and families facing poverty, housing instability, and social isolation, delivered in a stigma-free setting and integrated with Hope House’s food and system navigation supports to prevent crises before they escalate. |
| Routes Connecting Communities Inc. | Accessible Routes to Health A compassionate, accessible-vehicle transportation program helping seniors, people with disabilities, and low income residents across York Region reliably reach medical appointments, reducing missed care and advancing health equity. |
| Canadian Mental Health Association, York Region Branch | Lead the Way: A Community Solution for Crisis Care The York Region Mental Health Community Care Centre will be a 24/7 mental health crisis hub with walk-in access for youth (12+), adults, and families, plus crisis stabilization and withdrawal-management beds. Its integrated, culturally responsive model aims to reduce ER visits and strengthen community resilience through early intervention. |
Together, these projects will reach some of Ontario’s most underserved populations — seniors living with dementia, youth in crisis, low income families, people with disabilities, and 2SLGBTQIA+ communities in rural and remote regions.
