The Accerta Health Access Foundation (the Accerta Foundation) improves the lives of those in need and actively gives back to the communities where we work and live. The Accerta Foundation amplifies the work that charitable organizations across Canada do to promote better health, independence, and productivity among people.
The Accerta Health Access Foundation provides a total annual grant of $150,000, which may be awarded to one or multiple applicants at the committee’s discretion. Through grants, we augment the work of registered charities that improve access to healthcare and help people in need live better lives.
The 2025 Accerta Health Access Foundation grant application period is now closed. Successful applicants will be notified within 98 days after the application deadline.

The Gender Affirming Services Clinic provides comprehensive gender-affirming clinical services for individuals within the 2SLGBTQIA+ community, with a focus on advancing health equity and reducing systemic barriers to care. It addresses significant service gaps in Northeastern and rural Ontario.
Community & Home Assistance to Seniors (CHATS)
CIBPA Place provides affordable, high quality overnight respite care for seniors living with dementia through short-term stays in a safe, home-like environment. The program supports participant well-being while providing caregivers with reliable relief to prevent caregiver burnout and crisis.
The Low-Barrier Mental Health Counselling for Community Members Experiencing Poverty project provides free mental health counselling for individuals and families facing poverty, housing instability, food insecurity and social isolation. The initiative aims to improve well-being by reducing barriers to timely and professional mental health support.
Routes Connecting Communities Inc.
The Accessible Routes to Health – Compassionate Transportation project helps seniors, people with disabilities and low-income residents across York Region access medical appointments through accessible transportation services that support health equity.
Canadian Mental Health Association, York Region Branch
Lead the Way: A Community Solution for Crisis Care provides a 24/7 mental health crisis hub for youth, adults and families. The centre offers integrated, culturally responsive support and aims to reduce emergency room visits, improve access to crisis care and strengthen community wellbeing through early intervention.
